I do not believe there are any forms to fill out but you would definitely want to have an identification system for keeping track of your locks. We attach a brass tag with a number onto our locks which also is a good way to track who should have the lock. If your employees are misplacing locks then I would have a serious discussion with them and start writing them up. If they are qualified to LOTO equipment then they had better be responsible enough to not lose the locks.
